    Purpose

The purpose of the Investigations Unit is to protect and enhance the public health, welfare, and safety of the residents and visitors of the State of Florida.   This is accomplished through  disciplining individuals, providers and training schools that do not comply with the statutes and rules regulated by the Bureau of EMS .

    Disciplinary Procedures & Actions

To determine discipline history of a particular license holder, you may use MQA's online Licensee Lookup System, which provides a simple "Yes" or "No" under the topic "Discipline on File" and the current status of the license.

Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.

For assistance with MQA's online system contact their Customer Service Unit at: (850) 488-0595 OR MedicalQualityAssurance@doh.state.fl.us
